HC Palliative Physician

The palliative care physician visit encompasses a thorough assessment of the patient in their own environment, allowing a more personal touch.
This visit addresses pain and other symptoms experienced by the patient .
All care plans will be made in consultation with the primary consultant. A care plan will then be shared with the patient & family detailing the treatment and costs involved .
